1. 1.1. You can choose to purchase GoLife Plus insurance plans ("GoLife Plus Plan") from Sun Life Malaysia Assurance Berhad Registration No. 199001005930 (197499-U) ("SLMA") via GoPayz website or GoPayz mobile application ("GoPayz") using your GoPayz account ("GoPayz Account"). We will deduct the monthly premium applicable for the GoLife Plus Plan you purchased from your GoPayz Account.

1.2. If you choose to buy GoLife Plus Plan using your GoPayz Account, the terms of the GoPayz Service Agreement, the GoPayz App and Web Terms of Use and the Privacy Notice which are made available at www.gopayz.com.my.

1.3. GoLife Plus Plan is subject to the terms of your agreement with the SLMA and SLMA's privacy policy which are made available on https://www.sunlifemalaysia.com/GOLIFE. U Mobile Services Sdn. Bhd. Registration No. 200401034375 (672884-H) ("U Mobile Services") makes no representations nor warranties on the GoLife Plus Plan.

2. Eligibility

 GoLife Plus Plan is made available to GoPayz customers only.
Â
3. Registration and Usage
Â
3.1.   You will need to:
Â
a. register for the GoLife Plus Plan by following the steps provided on GoPayz; and
Â
b. select either one of the following payment methods to pay for your GoLife Plus Plan premium:
i. via GoPayz Account where you must ensure that there is sufficient balance in your GoPayz Account for the premium chargeable; or
ii. via your debit or credit card linked to your GoPayz Account subject to the terms and conditions imposed by your card issuer. If you chose to pay your premium via your debit or credit card, you must ensure that:

(1) the premium chargeable does not exceed your credit card limit, for credit cards; or
Â
(2) you have sufficient balance in your debit card account to remit the premium chargeable.
Â
We make no representation nor warranties on the payment made via your debit or credit card.

3.2. We may stop allowing you to purchase or renew the GoLife Plus Plan using your GoPayz Account, for any reason whatsoever, with or without notice including if we suspect that there is fraudulent or unauthorized use or if our agreement with SLMA has been terminated or for any reason or for any operational reasons.

3.3.  You will not be able to purchase or renew your GoLife Plus Plan if your GoPayz Account is terminated or suspended for any reason. Your existing GoLife Plus Plan will remain valid until the expiry date provided on your e-certificate. Refunds are not applicable once your GoPayz Account is terminated.

3.4.   GoLife Plus Plan is not under our control and we are not responsible for the underwriting, claim process and any dispute resolution under the GoLife PlusPlan. If you have any issues relating the following matters in relation to your GoLife Plus Plan, you may contact GoPayz Customer Careline at +6018-388 3388:

* purchasing;
* premium payment;
* monthly renewals;
* termination; and
* refund.

3.5. If you have any other issues not listed in paragraph 3.4 above, you are required to contact SLMA directly for assistance.

4. Charges and Payment
 Â
 4.1.  We do not set the insurance premium payable for GoLife Plus Plan. The insurance premiums are determined by SLMA and are subject to adjustment or taxes determined by SLMA.

 4.2. When you purchase the GoLife Plus Plan using your GoPayz Account via:
Â
* GoPay Account, the premium payable shall be deducted from your GoPayz Account;
* credit card, the premium payable shall be chargeable to you credit card; or
* debit card, the premium payable shall be deducted from your debit card account.
    on a monthly basis unless you terminate your GoLife Plus Plan in accordance with the terms and conditions applicable.

4.3. You can only purchase one basic GoLife Plus plan (without any riders) at a time. You will be able to purchase the GoLife PLUS Plan as long as you have sufficient balance in your GoPayz Account or subject to the debit or credit card limit that has been linked to your GoPayz Account (if you elect to pay for your GoLifePlus Plan premium using your debit or credit card).

 4.4. You are advised to take note of the premium due date as prescribed on your GoLife Plus Plan e-certificate and the termination option available at the time of your purchase of the GoLife Plus Plan. You may terminate the GoLife Plus Plan via GoPayz or by contacting GoPayz Customer Careline at +6018-388 3388 ("GoPayz Customer Careline"). The termination steps are provided on www.gopayz.com.my for your reference.

4.5.  Requests for refund on the GoLife Plus Plan can be made by contacting GoPayz Customer Careline or via GoPayz subject to SLMA's terms and refund policies. The refund will be credited to your GoPayz Account or your debit/credit card based on your latest premium payment method if you meet the requirement of the refund policies imposed by SLMA.

5. General
Â
5.1. U Mobile Services may change any of these terms of use from time to time. Any such changes will be posted on GoPayz. It is your responsibility to check the terms of use.

5.2. By purchasing GoLife Plus Plan, you: -
Â
a. consent for U Mobile Services to collect, record, hold, store, use and disclose your personal information for purposes which are necessary or related to GoLife Plus Plan;
Â
b. agreed that your personal information shall be used by SLMA for the purpose of processing GoLife Plus Plan application; and
Â
c. acknowledge that our Privacy Notice at www.gopayz.com.my applies to how we use your personal information.

5.3. U Mobile Services does not own the copyright, trademarks, services marks or other marks of SLMA. These marks are owned and registered by their respective owners.
Â
5.4. These terms of use are governed by the laws of Malaysia. You agree that the Courts of Malaysia have jurisdiction over all matters arising from these terms of use.
