1.1 You may purchase GoLife 10 insurance plan (“GoLife 10 Plan”) from Sun Life Malaysia Assurance Berhad Registration No. 199001005930 (197499-U) (“SLMA”) via either GoPayz website or GoPayz mobile application (“GoPayz”) using your GoPayz account (“GoPayz Account”).
1.2 If you choose to purchase GoLife 10 Plan using your GoPayz Account, the terms of the GoPayz Service Agreement, the GoPayz App and Web Terms of Use and the Privacy Notice which are made available at https://www.gopayz.com.my
1.3 GoLife 10 Plan is subject to the terms of your agreement with the SLMA which is made available on https://www.sunlifemalaysia.com/GOLIFE and SLMA’s privacy policy which is made available at https://www.sunlifemalaysia.com/SunLife/media/SunLifeMedia/PDF/PrivacyNotice.pdf. U Mobile Services Sdn. Bhd. Registration No. 200401034375 (672884-H) (“U Mobile Services”) makes no representations nor warranties on GoLife 10 Plan.
2 Eligibility
GoLife 10 Plan is made available to GoPayz customers only
3 Registration and Usage
3.1 You will need to:
(i) register for the GoLife 10 Plan by following the steps provided on GoPayz; and
(ii) select either one of the following payment methods to pay for your GoLife 10 Plan premium:
(a) via your GoPayz Account where you must ensure that there is sufficient balance in your GoPayz Account for the premium chargeable; or
(b) via your debit or credit card linked to your GoPayz Account subject to the terms and conditions imposed by your card issuer. If you chose to pay your premium via your debit or credit card, you must ensure that:
(1) the premium chargeable does not exceed your credit card limit; or
(2) you have sufficient balance in your debit card account to remit the premium chargeable.
We make no representation nor warranties on the payment made via your debit or credit card.
3.2 We may stop allowing you to purchase or renew the GoLife 10 Plan using your GoPayz Account, for any reason whatsoever, with or without notice including if we suspect that there is fraudulent or unauthorized use or if our agreement with SLMA has been terminated or for any reason or for any operational reasons.
3.3 You will not be able to purchase or renew your GoLife 10 Plan if your GoPayz Account is terminated or suspended for any reason. Your existing GoLife 10 Plan will remain valid until the expiry date provided on your e-certificate. Refunds are not applicable once your GoPayz Account is terminated or suspended.
3.4 GoLife 10 Plan is not under our control and we are not responsible for the underwriting, claim process and any dispute resolution under the GoLife 10 Plan. If you have any issues relating the following matters in relation to your GoLife 10 Plan, you may contact GoPayz Customer Careline at +6018-388 3388:
(a) purchasing;
(b) premium payment;
(c) monthly renewals;
(d) termination; and
(e) refund.
3.5 If you have any other issues not listed in paragraph 3.4 above, you are required to contact SLMA directly for assistance via phone call to 1300-88-5055 or e-mail: to wecare@sunlifemalaysia.com.
4 Charges and Payment
4.1 We do not set the insurance premium payable for GoLife 10 Plan. The insurance premiums are determined by SLMA and subject to adjustment or applicable taxes.
4.2 When you purchase the GoLife 10 Plan using your:
(a) your GoPayz Account, the premium payable will be deducted from your GoPayz Account;
(b) where your credit card is linked to your GoPayz Account, the premium payable will be chargeable to your credit card; or
(b) where your debit card is linked to your GoPayz Account, the premium payable will be deducted from your debit card account,
on a monthly basis unless you terminate your GoLife 10 Plan in accordance with the terms and condition applicable.
4.3 You can only purchase one GoLife 10 Plan at a time. You will be able to purchase the GoLife 10 Plan as long as you have sufficient balance in your GoPayz account or subject to the debit or credit card limit that has been linked to your GoPayz Account (if you elect to pay for your GoLife 10 Plan premium using your debit or credit card).
4.4 You are advised to take note of the premium due date as prescribed on your GoLife 10 Plan e-certificate and the termination option available at https://www.gopayz.com.my at the time of your purchase of GoLife 10 Plan. You may terminate GoLife 10 Plan via GoPayz or by contacting GoPayz Customer Careline at +6018-388 3388 (“GoPayz Customer Careline”)
4.5 Request for refund on the GoLife 10 Plan during the free look cancellation period as provided in the GoLife 10 Plan terms, can be made by contacting GoPayz Customer Careline or via GoPayz subject to SLMA’s terms and refund policies. The refund will be credited to your GoPayz Account or your debit/credit card based on your latest premium payment method if you meet the requirement of the refund policies imposed by SLMA.
5 General
5.1 U Mobile Services may change any of these terms of use from time to time. Any such changes will be posted on GoPayz. It is your responsibility to check the terms of use.
5.2 By purchasing GoLife 10 Plan, you: -
(i) consent for U Mobile Services to collect, record, hold, store, use and disclose your personal information for purposes which are necessary or related to GoLife 10 Plan;
(ii) agree that your personal information shall be used by SLMA for the purpose of processing GoLife 10 Plan application; and
(iii) acknowledge that our Privacy Notice at www.gopayz.com.my applies to how we use your personal information.
5.3 U Mobile Services does not own the copyright, trademarks, services marks or other marks of SLMA. These marks are owned and registered by their respective owners.
5.4 These terms of use are governed by the laws of Malaysia. You agree that the Courts of Malaysia have jurisdiction over all matters arising from these terms of use.
