1. This GoPayz GoLife 10 Perlindungan Tenang RM75 Voucher Programme (“Programme”) is jointly organised by U Mobile Services Sdn Bhd (“U Mobile Services”) and Sun Life Malaysia Assurance Berhad (“SLMA”) which is an initiative by the Government of Malaysia to offer an incentive voucher worth RM75 to eligible recipient of Bantuan Keluarga Malaysia (“BKM”) for the purchase of Perlindungan Tenang Insurance product.
2. This Programme commences at 08:00 on 30 May 2022 and ends at 23:59 on 31 December 2022. (“Programme Period”).
3. By participating in this Programme, you agree to be bound by all the terms and conditions below (“Terms and Conditions”).
4. Programme Eligibility
a. This Programme is open to new and existing GoPayz users with active GoPayz accounts who:
(i) are recipients of BKM based on Lembaga Hasil Dalam Negeri’s latest records;
(ii) are Malaysian citizens or Permanent Residents between the ages of 21 to 49; and
(iii) are in good health condition.
GoPayz accounts are active if the subscriber is able to perform transactions using the GoPayz account.
b. U Mobile Services and SLMA reserve the right to reject any application by any participant without assigning any reason.
5. Programme Mechanics
a. To participate in this Programme, you must register GoLife 10 (“GoLife 10”) using your GoPayz account (“Opt-In”) during the Programme Period by:
(i) selecting the ‘Life Insurance” tab under Financial Services tab;
(ii) selecting GoLife 10; and
(iii) keying in “PTV75” under Promo Code column and proceed to click the confirm button at the checkout page.
b. GoLife 10 is underwritten by SLMA and subject to the applicable terms and conditions imposed by SLMA which are available at https://www.gopayz.com.my/en/support/terms-and-conditions/golife-10. U Mobile Services makes no representations or warranties on GoLife 10.
c. Each eligible subscriber is only entitled to Opt-In once during the Programme Period.
d. GoLife 10 will be automatically renewed on a monthly basis upon the end of Incentive Period (as defined in paragraph 6(b) below) unless you opt-out by contacting GoPayz customer careline at +6018-388 3388 or SLMA’s customer careline at 1300-88-5055 (“Opt-Out”).
a. Participants who complete the steps listed in paragraph 5(a) above will receive an incentive voucher worth RM75.00 (“Incentive Voucher”).
b. The Incentive Voucher automatically entitles the participants to receive GoLife 10 insurance coverage at no cost for a period of 8 months from the date of the participants’ receipt of the GoLife 10 insurance e-certificate from SLMA (“Incentive Period”)
c. The monthly premium payable for GoLife 10 is RM10 (“Premium”). However, during the Incentive Period, you will not be required to remit the Premium.
d. The Premium will be charged in the following manner:
(i) if you opt to pay for the Premium via your GoPayz Account, the Premium payable will be deducted from your GoPayz Account;
(ii) if you opt to pay for the Premium via your credit card that is linked to your GoPayz account, the Premium payable will be charged to your credit card; or
(iii) if you opt to pay for the Premium via your debit card that is linked to your GoPayz account, the premium payable will be deducted from your debit card account
on a monthly basis unless you Opt-Out in accordance with the policy terms prescribed by SLMA.
a. U Mobile Services or SLMA may change any of these Terms and Conditions (including the terms and conditions applicable to GoLife 10) and may terminate, suspend this Programme or extend the Programme Period. Any such change will be announced on our website or the website of SLMA. It is your responsibility to check these Terms and Conditions on our website and the website of SLMA and if you do not agree to any of the changes, you must immediately cease participation in the Programme.
b. By participating in the Programme, all participants:
(i) consent for U Mobile Services to collect, record, hold, store, use and disclose their personal information for purposes which are necessary or related to the participation in the Programme;
(ii) consent for U Mobile Services to disclose or publish their personal information such as their names and identities and any general information that U Mobile Services sees fit about the participants in any media, marketing or advertising materials;
(iii) grant U Mobile Services the absolute and unrestricted right to modify, use and publish any still or moving images of the participants for any promotional, marketing, commercial or other related purpose, without any payment or compensation; and
(iv) agree that their personal information will be used by U Mobile Services and SLMA for the purpose of processing GoLife 10 application.
c. To the extent permitted by law, U Mobile Services and its related corporation (as defined under Companies Act 2016) (“Related Corporation”) exclude all liability in connection with this Programme. All participants agree to release U Mobile Services and its Related Corporation from and indemnify U Mobile Services and its Related Corporation against all liability (including negligence) for any personal injury, for loss or damage (including loss of opportunity) or breach of any of these Terms and Conditions; whether direct or indirect, special or consequential arising out of participation in this Programme.
d. The subscription and use of GoPayz is subject to the GoPayz Service Agreement and where applicable, these Terms and Conditions. In the event of any inconsistency, GoPayz Service Agreement Terms and Conditions will prevail to the extent of such inconsistency.
e. The Programme and these Terms and Conditions are governed by the laws of Malaysia. The participants agree that the Courts of Malaysia have jurisdiction over all the matters arising from this Programme.
